Under Arizona law, the Ombudsman Office at the Industrial Commission of Arizona is established to provide information about the workers' compensation system and rules governing commission proceedings and to assist in clarifying the methods used to determine a person's workers' compensation benefits. The Ombudsman Office is not permitted to provide legal advice. During a workers’ compensation claim, you may be required to file certain forms with the Claims Division at the Industrial Commission of Arizona. These forms can be filed electronically with the Claims Division. Available forms can be reviewed at https://www.azica.gov/forms .
