This form can be used to order copies of OSHA Standards Books, 29 CFR Part 1910 (General Industry) or 29 CFR Part 1926 (Construction). These standards, enforced by the Arizona Division of Occupational Safety and Health (ADOSH), are substantially the same as the federal OSHA standards which can be accessed on line at https://www.osha.gov/law-regs.html or https://www.osha.gov/. 

Payment may be made by credit card, personal check or money order by filling out the appropriate sections of the form and providing complete and accurate payment information.

Checks or money orders must be made payable to: INDUSTRIAL COMMISSION OF ARIZONA

Carefully follow the directions on the form and return it as instructed.   This form must be completed in its entirety.  Failure to do so may cause a delay in processing.
